Transitions Championship, a PGA TOUR® Event
OFFICIAL PROMOTION/SWEEPSTAKES RULES
1. NO PURCHASE NECESSARY TO ENTER OR TO WIN: A purchase will not increase your chances of winning. The Promotion/Sweepstakes (hereafter referred to as Promotion) is subject to all Federal, State, Local and Municipal laws and regulations. Void where prohibited by law.
2. SPONSOR: The Promotion is sponsored by Transitions Optical, Inc., 9251 Belcher Road, Pinellas Park, FL 33782.
3. ELIGIBILITY: The promotion is open only to legal residents of the United States, excluding Puerto Rico and protectorates, who are eighteen (18) years of age or older at time of entry. Promotion is not open to employees and directors of the Sponsor or any of their respective parent companies, affiliates, subsidiaries, advertising or promotional agencies. Sweepstakes judges and legal advisors, or to the immediate family members of, or to any persons domiciled with, any such person. All winners are subject to Sponsor's verification of eligibility hereunder.
4. PROMOTIONAL PERIOD: The promotion will begin at 12:01 a.m. (EST) on January 22, 2010 and ends February 26, 2010. Entries received outside of the entry period will be disqualified.
5. HOW TO ENTER:
A. Visit a participating retail location: Request a demonstration of Transitions® lenses. Upon Completion of your demonstration, you will receive an entry form for you to complete. Please hand your completed entry to the demonstrator. You are now entered into the Sweepstakes. For a listing of participating retail locations, please visit www.ecca.com.
B. Enter by Mail: To enter by mail, submit an official entry form, available at participating retail locations or hand-print a 3" x 5" piece of paper with your name, address, city, state, zip code, daytime phone number (where you can be reached during normal business hours), birth date and the words TRANSITIONS PGA TOUR® Sweepstakes, 9251 Belcher Road, Pinellas Park, FL 33782. No photocopies, mechanical reproductions, altered illegible, incomplete, mutilated, postage due or forged entries will be accepted. Sponsor is not responsible for lost, mutilated or misdirected mail. All mail-in entries must be postmarked by February 21, 2010 and received by February 26, 2010.
ALL OTHER MAIL-IN ENTRIES ARE NULL AND VOID.
SWEEPSTAKES PRIZES: Grand Prize: An all-inclusive trip to the Transitions Championship, a PGA TOUR® Event in March 2010. Winner also receives a spot to play in the Pro-Am or be an Honorary Observer (depending on handicap). Prize details broken down below:
- Roundtrip airfare for two
- Five days/Four nights hotel accommodations (Mar 16-20, 2010)
- Two tickets daily to the Transitions Championship, a PGA TOUR® Event
- Rental car
- $100 Daily stipend
- Pro-Am spot in tournament (or Honorary Observer)
Prizes are non-assignable and non-transferable. The winner is solely responsible for payment of any and all federal, state or local taxes pertaining to their prize. Odds of winning dependent upon the number of eligible entries received. ALL SWEEPSTAKES PRIZES WILL BE AWARDED.
6. GRAND PRIZE DRAWING: One Grand Prize will be selected in a random drawing on or about 3-1-2010. A drawing will be conducted from all eligible entries received to date. Transitions Optical will attempt to contact the selected Grand Prize Provisional Winner in person by telephone during regular business hours (9:00 a.m. and 5:00 p.m. EST) using the telephone number provided on the entry form. If Transitions Optical is unable to reach a Grand Prize Provisional Winner in person by telephone (Transitions Optical will call only once and no messages will be left), that Grand Prize Provisional Winner will be disqualified and a substitute Grand Prize Provisional Winner will be randomly selected from the remaining eligible entries. The first eligible Grand Prize Provisional Winner Transitions Optical successfully contacts in person by telephone and who claims the Grand Prize in accordance with these Official Rules will be declared the Grand Prize Winner.
The winner will be required to claim their prize by completing and signing an affidavit of eligibility, a liability release form and, except where prohibited by law, a publicity release, which must be returned via mail within four (4) business days of notification postmark. In the event of non-compliance with this time period, an alternate winner will be selected with no further compensation. If notification of prize award is returned to sponsor as undeliverable, the prize will be awarded to an alternate winner with no further compensation.
Winner may waive the right to recieve a prize. The return of any prize or prize notification as undeliverable will result in disqualification, the forfeiture of all interest in the prize, and selection of a substitute winner from among all remaining eligible entries. Unless prohibited by law, acceptance of a prize constitutes permission for the Sponsor and its agencies to use the Winner's name, address (city and state) and/or likeness for advertising and trade purposes without further compensation, worldwide, and in perpetuity, in any and all forms of media, now known and hereafter devised including without limitation, the internet.
7. DISCLAIMERS: By entering, participants acknowledge and agree that the Sponsor, Transitions Optical, Inc. and their respective parents, subsidiaries, affiliates, franchises, advertising and promotional agencies and the officers, directors, employees, agents and representatives of each of the foregoing companies (collectively, the "Promotion Entities"), will not be liable for losses or injuries of any kind resulting from acceptance/possession/use and/or misuse of prize, travel to and from any prize-related activity or participation in the prize to be awarded. Participants release and agree to hold harmless Promotion Entities from any and all liability from any of the claims described above.
8. SWEEPSTAKES WINNERS LIST: For the name and hometown of the Grand Prize Winners (available after 4-1-2010), and/or a copy of these Official Rules, send a self-addressed, stamped business-sized envelope to TRANSITIONS PGA TOUR® Promotion, 9251 Belcher Road, Pinellas Park, FL 33782. Attn: Tracy Jones. Indicate whether you are requesting the Winner List or a copy of the Official Rules, or any combination of the foregoing. Residents of Vermont may omit return postage.
